Practice Setting: Surgery Center

Assignment Details
- Start Date: March 30, 2026
- Contract Length: 1 week (3/30–4/3)
- Schedule: Monday–Friday
- Hours: 7:00 AM – 3:00 PM
- Call: None

Clinical Details:
- Coverage Reason: Vacation coverage
- Practice Model: Independent practice – no supervision
- Case Mix: Cataracts, GI, Podiatry, Orthopedics, Regional anesthesia, Some Pediatrics

Requirements:
- Active Indiana CRNA license (must be in hand at submission)
- Comfortable working independently
- Ability to cover full assignment dates

Credentialing
- Estimated timeline: ~2 weeks
