Job description

Start Date: May 2026 or as soon as credentialed
Contract Length: 3 months with option for extension
Schedule: 8-hour Shifts 0700 - 1530. No weekends, No Call
Specific Schedule to start:
May 4-6, 18-21, 26-29
June 1, 3, 8, 9, 10,15
EMR: TBD
Special procedures: Ortho, Total Joints and Sports Medicine

Active Indiana License
Board Certified
Credentialing Timeframe: TBD
Medical malpractice insurance provided
