Start Date: August 2026 or As soon as credentialed
Schedule: Full time, 3+ weeks per month, 4x8s or 4x10s
EMR: HST
Special procedures: Ophthalmology, Cataract, Oculoplastic, Retina

Indiana License Required
Board Certification Required
Credentialing Timeframe: 45-60 Days, Temp privileges for clean files available!
Medical malpractice insurance, travel, and lodging provided
